Bridge-Enhanced ACL Repair (BEAR®) Surgery for Anterior Cruciate Ligament (ACL) Regeneration

Bridge-Enhanced ACL Repair (BEAR®) Surgery for Anterior Cruciate Ligament (ACL) Regeneration

Introduction to ACL Repair and Regeneration with BEAR®  Technology

ACL tears have traditionally meant long recovery times, uncertain returns to peak performance, and the prospect of arthritic knee degeneration later in life. Standard treatments, such as ACL reconstruction with a tendon graft, though effective, are not a perfect solution and can be associated with a number of problems and limitations. Bridge-Enhanced ACL Restoration (BEAR®) to facilitate ACL repair and regeneration marks a significant advancement in ACL treatment, leveraging the body’s natural healing capabilities to restore the native ACL.

Challenges of Traditional ACL Treatments

ACL injuries are common among athletes and active individuals, and in the early days of orthopedics, primary repair involved suturing the torn ends of the ACL with the hope of natural healing. However, the knee joint’s synovial fluid, while essential for lubrication, often impeded this process by washing away crucial healing cells and proteins, leading to a high failure rate for these repair attempts.

This challenging environment made it difficult for the ligament to heal properly, necessitating the development of more effective treatment methods to address the shortcomings of primary ACL repair.

The Shift to ACL Replacement with Reconstruction

Advancements in ACL treatment shifted the focus from primary repair to reconstruction, where the torn ligament is removed entirely and replaced with a tendon graft, either from the patient (autograft) or a tissue donor (allograft). While this technique has been effective, it comes with challenges and shortcomings, and not all patients are able to return to their sport and their previous level of performance. ACL reconstruction has also been associated with prolonged rehabilitation, chronic pain and weakness and potential complications like graft failure and painful arthritis, highlighting the need for alternative approaches like the BEAR® resorbable implant which allows preservation of the native ACL.

Limitations of ACL Reconstruction

  1. Lack of Proprioception and Sensory Feedback: Reconstructed ACL graft tissue lacks the original ligament’s nerve supply. This lack of innervation is thought to diminish knee position awareness and possibly increase the risk of re-injury.
  2. Graft Integration Issues: Successful graft integration varies due to biological factors, graft type, and surgical precision, potentially leading to suboptimal results.
  3. Risk of Osteoarthritis: Post-reconstruction, abnormal biomechanical joint stress and lack of position sense may promote osteoarthritis development.
  4. Re-injury and Long-Term Performance: High re-injury rates and performance issues persist, especially among high-impact activity participants.
The BEAR Implant is indicated for a broad range of ACL tear types.

How BEAR® ACL Surgery Works

Preoperative Preparation

A thorough evaluation, including imaging studies and patient assessment, ensures BEAR® surgery is suitable. Not every ACL patient is a candidate. The key requirement is that the ACL must not be so severely damaged or chronically torn that no viable tissue remains to repair and heal.

Surgical Procedure

Performed minimally invasively, BEAR® surgery involves:

  1. Arthroscopy: Small incisions are made to insert an arthroscope to visualize the inside of the knee.
  2. Primary ACL Repair: All viable tissue in the injured ACL is retained. Any scar tissue or adhesions restricting the ACL tissue are released. The torn ACL tissue is then reapproximated and arthroscopically stitched back in place with high-strength suture so it can heal in the proper position.
  3. Placement of the BEAR® implant: The repaired ligament is then covered with the patented BEAR® resorbable collagen implant that bridges across the repair site. This implant is enriched with the patient’s blood or platelet concentrate, providing a protective, healing environment. Eventually, the collagen implant is absorbed and replaced by the growth of new ligament tissue.

Postoperative Care and Rehabilitation

After surgery, the postoperative rehabilitation program begins, focusing on restoring knee flexibility, strength and function. Patients tend to have less pain and often reach postoperative milestones sooner after ACL repair versus reconstruction.

Dr. Meier’s extensive experience with knee rehabilitation ensures each patient receives a customized recovery plan, maximizing their return to sports, fitness, and daily activities.

The Advantages of BEAR® ACL Surgery

  1. Preservation of Native Ligament: BEAR® surgery retains the original ligament tissue, preserving its nerve supply and proprioceptive function. This may help improve knee stability and reduce the risk of re-injury.
  2. Enhanced Regeneration: The resorbable collagen implant in BEAR® surgery bridges any tissue gap and supports natural ACL healing, restoring the ligament to near-original quality. It acts as a scaffold that promotes tissue integration and regeneration.
  3. Faster Recovery: The less invasive nature of BEAR® surgery leads to quicker rehabilitation and a faster return to activity. Patients experience less postoperative pain and a more rapid restoration of knee function.
  4. Improved Patient Satisfaction: Patients undergoing BEAR® surgery report higher satisfaction due to quicker recovery and better knee function. Early data suggest they may be more likely to regain their pre-injury activity levels. Interestingly, patients who have experienced both traditional ACL reconstruction in one knee and BEAR® ACL repair in the other almost universally report a preference for their BEAR® knee.
  5. Reduced Osteoarthritis Risk: By maintaining the native ligament, BEAR® surgery may better preserve natural knee mechanics, possibly reducing joint stress and cartilage wear. Early evidence suggests that this may lower the long-term risk of developing osteoarthritis, a problem that has been associated with ACL reconstruction.

Comparing BEAR®  ACL Repair to Traditional ACL Reconstruction

For decades, ACL reconstruction has been the standard treatment for ACL tears, but it presents challenges, including more invasive surgery, longer recovery times and the need for graft integration. BEAR® ACL surgery offers a new approach that preserves the native ligament and promotes natural healing and ligament regeneration.

Patients who wish to retain their original ligament structure, recover more quickly, and reduce the risk of post-surgical complications may benefit from ACL BEAR® surgery over traditional reconstruction.

Who Is an Ideal Candidate for BEAR® ACL Surgery?

  • BEAR® ACL repair is a viable treatment option for many patients, though certain factors determine eligibility. This procedure is most effective for individuals who: have an ACL injury with sufficient remaining tissue for repair. If the ACL tissue has been too severely damaged or if so much time has gone by that the ligament tissue has deteriorated extensively, primary repair may not be feasible.
  • Active individuals seeking a quicker recovery with a more natural healing process
  • Those who prefer a less invasive procedure over graft-based reconstruction
  • Active people who like the idea of a more natural-feeling knee.

Patients hoping to undergo ACL BEAR® surgery should undergo a comprehensive evaluation by a surgeon to perform this newer technique to determine whether they are suitable candidates. A clinical assessment with imaging studies will help guide the best course of treatment.

Rehabilitation and Recovery Timeline

Recovery after BEAR® ACL surgery is a gradual process focused on restoring mobility, strength, and stability while the ligament heals. For most patients, ACL recovery time means getting back to light activities within a few weeks, with a full return to sports often taking 9–12 months.

Since every patient heals differently and rehabilitation programs continue to evolve, your surgeon and physical therapist will guide you through a plan tailored to your needs. BEAR® ACL surgery supports natural healing and may provide a smoother recovery compared to traditional ACL reconstruction.

Why More Athletes Are Choosing BEAR® ACL Repair

Athletes, from amateur to professional, are increasingly selecting BEAR® ACL repair due to its advantages expediting recovery and preserving long-term knee function.

A key benefit of BEAR® ACL surgery is the ability to maintain proprioception, which plays a critical role in balance, coordination, and athletic performance. By allowing the ACL to heal and regenerate rather than removing and replacing it with a graft, BEAR® ACL repair minimizes disruption to natural biomechanics, potentially reducing the risk of long-term complications such as osteoarthritis.

In addition to structural benefits, BEAR® ACL repair can offer a shorter rehabilitation timeline, allowing athletes to return to training and competition faster than with traditional reconstruction. This is a crucial factor for individuals who need to regain high-level function in a time-sensitive manner.
